Enhancing the Health of the Underprivileged in Iran (ESMA)
The Alavi Foundation, within the framework of higher-order policies and in fulfillment of its mission to enhance the health of the underprivileged—emphasizing preventive measures and addressing critical bottlenecks—has adopted the “Executive By-law for Promoting the Health of Iran’s Underprivileged,” abbreviated as ESMA, as its roadmap.
This by-law serves as an operational guide for health sector implementers and stakeholders. It outlines a variety of executive strategies that help identify and meet the needs of the target population. Through active social participation, empowerment of volunteer (jihadi) groups, and collaboration with community-based organizations, it facilitates the improvement of health standards and access to appropriate healthcare and medical services for residents of deprived and rural areas.

Major Completed Plans and Projects
Health promotion programs at the Alavi Foundation began in 2008 with maternal health initiatives in cooperation with the Office for Nutrition Health Improvement, Deputy, Ministry of Health and Medical Education. The foundation has since implemented a broad range of programs, including establishing and equipping health centers and ensuring food security in underprivileged areas.

Maternal Health Program
Nutritionally at-risk pregnant and breastfeeding women in rural areas were identified with the help of local health offices. From the fourth month of pregnancy through the sixth month of breastfeeding, participants received food baskets every two months—contents approved by Ministry experts. These visits also facilitated regular maternal and infant check-ups and testing.

From 2014
The foundation supported the construction and equipping of healthcare facilities in areas affected by war or lack of access. Projects include:
240-bed Abu Dhar Children’s Hospital in Ahvaz
242-bed Ayatollah Taleghani Hospital in Abadan
64-bed hospital in Maraveh Tappeh
Phase I of Ya Zahra Hospital in Dezful

Since late 2016
The foundation signed an agreement with the Ministry of Health to build 39 24-hour comprehensive health centers in underprivileged areas with 50% cost sharing.
